How Does UV Fluorescence Dermoscopy Work?

A conventional dermatoscope uses magnification and controlled illumination to reduce the effect of skin-surface reflection and make structures that are difficult to see with the unaided eye more visible. A dermatoscope with UV light adds another observation pathway: ultraviolet-induced fluorescence dermoscopy, usually abbreviated as UVFD.

The principle is different from ordinary reflected-light dermoscopy. With UVFD, ultraviolet radiation reaches fluorescent substances, or fluorophores, within or on the surface of the skin. These substances absorb the excitation energy and can then emit visible light at a longer wavelength. This wavelength shift is the basis of the fluorescence seen during UV examination.

The final UV image is influenced by more than the lesion itself. UV wavelength determines which substances are effectively excited. Ambient light can reduce fluorescence contrast, which is why fluorescence examinations are generally easier to interpret in a darkened environment. Products on the skin are another important variable. Makeup, moisturizers, topical medication, soap residue, fibers, and other materials may fluoresce and create findings unrelated to the underlying skin.

The DE-5100 concentrates on a relatively small area but provides higher magnification and allows fluorescence to be compared with several other illumination modes. The DE-315 serves a different purpose. Its larger lens helps the user examine fluorescence across a wider area, and its illumination includes both 365 nm and 405 nm. A conventional polarized dermatoscope such as the DE-3100 focuses instead on structural information produced by polarized and non-polarized visible light.

Polarization also works by a different physical principle. Cross-polarized dermoscopy suppresses much of the directly reflected surface light and improves visualization of deeper structures, while non-polarized dermoscopy tends to preserve more superficial epidermal information. The two techniques provide complementary rather than identical images.

Is a UV Dermatoscope or a Separate Wood’s Lamp More Suitable?

The decision depends mainly on observation area and workflow.

A multi-light dermatoscope places several optical modes in one handheld instrument. A user can examine a localized area with conventional dermoscopy, switch to 365 nm UV, and capture a magnified image without moving to another device. This can simplify workflows where the main requirement is detailed examination and documentation of selected sites.

Its limitation is field size. Even a dermatoscope with a relatively large viewing aperture examines a much smaller area than a dedicated Wood’s lamp. A Wood’s lamp is therefore more suitable when the first task is to observe the distribution of fluorescence across a broader region of skin, scalp, or hair. The correct choice is therefore not based on whether one technology is universally better. It depends on whether the user mainly needs broad fluorescence observation, close-up UV dermoscopy, or both.

 

What Should Buyers Look for When Selecting a Dermatoscope with UV Light?

UV wavelength should be one of the first specifications checked because fluorescence depends on excitation wavelength. Around 365 nm is widely associated with long-wave UVA fluorescence examination and is also commonly used in current UVFD research.
